Psychological Evaluation

Every evaluation is customized to fit your specific needs, referral questions, and goals. Because of this, the cost depends on factors such as the scope of testing and the type of results you’d like (for example, a formal report or a therapeutic story/letter). Typically, evaluations range from $3,200 to $3,500.

Insurance

Evergrow is an out-of-network provider and does not currently accept insurance. Upon request, we offer superbills that you may present to your insurance for potential reimbursement.

Good Faith Estimate Notice

Under the No Surprises Act (H.R. 133), health care providers must give clients or patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services. This is called a “Good Faith Estimate” (GFE).

  • A GFE helps clients estimate the expected charges they may be billed for receiving certain health care items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

  • A GFE is not a contract and is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of therapy visits. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
